Head to Health Mental Health Hubs
Head to Health is an integrated mental health service that finds the best support for people, families and carers who may be experiencing distress or mental health issues. The service can be accessed via phone, or by visiting one of three Hubs located across the Eastern Melbourne Primary Health Network (EMPHN) region.

Head to Health is available for referrals, advice and support. These services provide access to a multidisciplinary mental health team and are designed to complement and supplement care from a GP.
Phone service
The Head to Health phone service (1800 595 212) is staffed by experienced mental health professionals who will listen to your needs and match you with support options tailored to your individual needs.
This may include:
- Connecting you with suitable services
- Directing you to follow-up care or support through a Head to Health hub (either onsite or via telehealth)
- Referring you to a specialist or acute mental health service (including emergency care)

Hub services
Head to Health Hubs provide access to multidisciplinary teams of mental health workers, including psychologists, nurses, social workers and peer workers. The Hubs provide a flexible model of care tailored to the person, recognising that people have different mental health care needs at different times.
Anyone has the ability to refer to receive Head to Health support.

There are three hub locations in the EMPHN region, including two hubs (Heidelberg West & Hawthorn) and one spoke site (Box Hill).

Information for GPs
Head to Health provides GPs with an additional option for managing the number of patients who are experiencing mental health issues at this time. The telephone service helps navigate to the appropriate level of mental health and wellbeing support, depending on a person’s needs and goals.
Even if GPs have great networks and well-used referral pathways, Head to Health provides an additional referral pathway, as outlined in the diagram.
