Partnerships and healthcare collaboratives
A key purpose of Eastern Melbourne Primary Health Network (EMPHN) is to bring together every part of the primary healthcare system and facilitate meaningful collaboration, to achieve system improvement and better health outcomes for our community.
The EMPHN Clinical Governance framework defines the importance of clinical governance; setting accountability, so that it is understood and enacted in all programs and services funded by EMPHN.
EMPHN Clinical Governance Framework
Primary healthcare collaboratives are region-wide platforms of service providers and organisations working towards shared goals, such as the management of chronic disease and complex conditions in people at risk of poor outcomes.
These collaboratives bring about improved alignment of primary and secondary service providers, with a shared objective of slowing disease progression. In turn, this can prevent deterioration and reduce avoidable hospital admissions through improved community-based models of care.
Our healthcare collaboratives
Currently, EMPHN collaborates with the Eastern Melbourne Health Alliance (EMHA). EMHA member organisations include: Eastern Health, EMPHN, EACH, Inspiro, and healthAbility. The Health Alliance’s vision is to help people in our community achieve their best health and wellbeing works to think and act as one system. They strive to implement a single service system that addresses past barriers to working together and is built around the needs of people in our community. The alliance serves the local government areas (LGAs) of Boroondara, Knox, Manningham, Maroondah, and Whitehorse as well as the majority of the Shire of Yarra Ranges. Included at their margins, but predominantly supported by other Primary Healthcare Networks and Alliances, are the Shires of Mitchell and Murrundindi and the City of Monash.
EMPHN also continues to engage regularly with local hospital networks to build a better integrated health system.
Mental health collaboratives
We actively collaborate with the Eastern Mental Health Service Coordination Alliance, the North Eastern Mental Health Service Coordination Alliance and the Eastern Regional Coordinators. You’ll find links to further information below. In addition, we are also working collaboratively with stakeholders in Northern Health and Monash Health service areas through existing structures. These collaboratives, and our work in these areas, will grow and develop over time.
