Join us for our upcoming Quality Improvement Webinar to discover how to use the Hospital Avoidance Tool (HAT) report in POLAR. This will help you pinpoint patients at urgent risk of hospitalisation, while boosting your practice’s completed General Practitioner Management Plans (GPMPs) or GPMP reviews for eligible patients.
