In this session, attendees will be introduced to a Quality Improvement (QI) activity using the Hospital Avoidance Tool (HAT) report in Polar to identify patients at urgent risk of hospitalisation and increase your number of completed General Practitioner Management Plan (GPMP) or review of GPMP for eligible patients.

This QI activity will also support continuity of care activities for identified patients by offering MyMedicare registration and uploading Shared Health Summaries.

Learning Outcomes:

At the end of the session, participants will be able to:

  • Update your team on hospitalisation risk, GP Management Plan (GPMP) or review of GPMP, MyMedicare and Shared Health Summaries.
  • Identify patients at urgent risk of hospitalisation using the Hospital Avoidance Tool (HAT) report in Polar.
  • Implement activities to increase number of completed GP Management Plan (GPMP) or review of GPMP for identified patients at urgent risk of hospitalisation.
  • Offer MyMedicare registration and Shared Health Summary upload for those identified patients at urgent risk of hospitalisation.

Attendees will be provided with the Identifying patients at risk of Hospitalisation Quality Improvement Toolkit, POLAR walkthroughs and Plan-Do-Study-Act examples.
