EMPHN Aged Care Virtual Communities of Practice

Following strong interest in the Aged Care Collaborative Sessions, EMPHN is launching quarterly Virtual Communities of Practice (CoPs).

These sessions are designed to support GPs and practice nurses visiting Residential Aged Care Homes (RACHs), along with facility staff and other providers involved in resident care. 

These one-hour online sessions provide a practical, peer-led forum to share approaches, discuss common challenges and strengthen coordination of care for residents. Each quarter will focus on a new theme shaped by participant priorities. Quarter 1 theme: Dementia/BPSD and restrictive practice.

Upcoming session dates and registration

Open to GPs, practice nurses, RACH staff and other providers supporting aged care residents. Participants are encouraged to register for the regional session most relevant to their work or location.
