Aged Care Virtual Community of Practice: South Region

These quarterly sessions provide a practical, peer-led forum for general practice, residential aged care and other healthcare providers to continue the conversations started through the Aged Care Collaborative Sessions. Each session is designed to support shared learning, strengthen local connections and explore practical approaches to improving coordination of care for residents.

Who should attend

This session is suitable for:

• General practice staff, particularly GPs providing care to residents in RACHs
• Practice nurses and nurse practitioners supporting aged care work
• RACH staff, including clinical managers, facility managers, and relevant corporate roles
• Other healthcare providers involved in delivering care to residents in RACHs

What to expect

Each quarter will have a different theme. This session will focus on Palliative Care / End of Life Care and difficult conversations including a case study and will also include opportunities for discussion, peer learning, shared problem-solving and resource sharing.
