EMPHN Aged Care Virtual Communities of Practice
Supporting connection between residential aged care homes and GPs
Eastern Melbourne Primary Health Network (EMPHN) is launching a series of virtual Aged Care Communities of Practice (CoP) to support ongoing collaboration across general practice, residential aged care, and allied health.
These sessions build on the strong engagement from recent Aged Care Collaborative Sessions, where participants highlighted a shared commitment to improving coordination, communication, and continuity of care.
The CoP provides a structured, ongoing forum for services to connect, share experiences, and work through practical challenges together.
About the CoPs
- Virtual sessions delivered regionally (North, East, South)
- Quarterly meetings to support sustained engagement
- Designed to be interactive with participant-led case study
- Focused on real-world challenges and priority topics identified by participants

- Health system partners e.g. Victorian Virtual ED, Ambulance Victoria, Resi In-Reach and geriatricians
