Eastern Melbourne Primary Health Network (EMPHN) works with residential aged care homes (RACHs) and aged care workers to improve access to high-quality healthcare for all aged care residents in our community.

EMPHN has received Department of Health and Aged Care (DoHAC) funding to assist with the implementation of two programs relevant to RACHs:

  • The General Practice in Aged Care Incentive (GPACI) and
  • My Health Record (MHR)

We are gathering information to help us in planning activities that best suit the needs of RACHs.
