Eastern Melbourne PHN invites expressions of interest to join a new evidence-based mental health model of care.

The Link-me+ model addresses feedback from local GPs, offering support to provide tailored mental health care according to patients’ prognosis and needs.

Developed by Eastern Melbourne PHN in collaboration with University of Melbourne, this model of care is grounded in patient experience and developed through extensive consultation and research.​

How does it work?

The model aims to provide accessible, patient-focused mental health support through:

  • A patient-facing digital prognostic tool to help GPs categorise mental health needs based on predicted symptom severity over time.
  • Dedicated ‘Care Navigators’ who collaborate with GPs to support patients in coordinating care, building patient self-efficacy, developing an action plan and finding the right services to suit their needs.
  • Ongoing GP support and upskilling to provide best practice in mental health care.

A localised health and social services directory will support both GPs and Care Navigators.

  • Fully supported implementation for GPs and Practices (including financial supports)
  • Support to optimise billing and improve practice efficiency
  • In-house education on priority mental health topics such as ADHD and eating disorders
  • Access to EMPHNs mental health referral support service, SupportConnect
  • Access to EMPHNs localised health and social service navigation tool
  • Provide feedback and contribute to the continuous improvement of the model
  • Network and collaborate with other GPs and general practices participating in the model
