SupportConnect website retiring – updated referral pathways for practices

The SupportConnect website is being retired and will no longer be maintained. To ensure patients continue to access timely and appropriate mental health support, practices are asked to stop referring patients to SupportConnect and update referral pathways now.

This change is designed to support better care by directing patients to current, reliable and up‑to‑date mental health services. Importantly, no one is being left without help, and clear alternatives are already in place.

What to do now

GPs and practice teams should redirect patients to one of the following options:

  • Medicare Mental Health phone line: 1800 595 212, available Monday to Friday, 8:30am to 5:00pm. This service provides timely support and guidance about appropriate mental health options.
  • Request a call back from the Medicare Mental Health team
    A call back can be requested via the EMPHN website
  • Find local services using EMPHN’s Service Finder
    EMPHN’s online Service Finder allows GPs, practice staff and patients to identify local mental health and wellbeing services.

Practices are encouraged to check for and remove any saved SupportConnect referral forms. The Medicare Mental Health referral form is now available within the Healthlink smart forms.

Using these referral options will help ensure continuity of care and maintain patient confidence during this transition.
