Request for Expressions of Interest: Unified Program

Throughout 2025, Eastern Melbourne Primary Health Network (EMPHN) has engaged in a series of co-design workshops with service providers, consumers, and key stakeholders to explore a new commissioning model—one that enables place-based, integrated care and fosters innovation.

The Unified Program represents a strategic shift in how primary health care is commissioned and delivered across the EMPHN catchment. We’re building something progressive: a connected, integrated model of care that strengthens over time.

Phase One: A call to innovators

Phase One focuses on four priority cohorts. EMPHN invites expressions of interest from service providers—whether solo organisations or collaborative partnerships—who are ready to deliver now and evolve with us as the model expands.

This is an opportunity to help co-create a future-ready system that is:

• Equity-driven – centering care around hardly-reached communities, including First Nations peoples, CALD and LGBTQIA+ populations, and those facing systemic disadvantage
• Integrated and place-based – delivering multidisciplinary care that reflects local needs and connects seamlessly with general practice and the broader health system
• Flexible and outcomes-focused – adapting to community strengths while being accountable for meaningful health outcomes
• Collaborative – forging strong partnerships across hospitals, general practice, equity and specialist providers, and community organisations
• Innovative – co-designing with consumers and using outcome-linked funding to test and scale new approaches
• System-oriented – strengthening governance, networks, and accountability to drive collective impact

To participate in the Request for Expressions of Interest (REOI) process, interested providers must be registered on eProcure. Register for eProcure here.

Once registered, you will receive email notifications about EMPHN opportunities published on the platform. 

REOI timelines: 

• Tuesday 21 October 2025, 2.00pm: REOI will be released via eProcure
• Tuesday 28 October 2025, 1.00pm – 2.00pm: REOI Online Industry Briefing. The briefing will be held via Microsoft Teams. Register to attend the industry briefing here. 
• Tuesday 11 November 2025, 2.00pm: REOI questions close
• Tuesday 18 November 2025, 2.00pm: REOI submissions will close via eProcure 

Industry briefing 

• EMPHN will hold an industry briefing session at 1.00pm – 2.00pm Tuesday 28 October 2025, following the REOI release. 
• The briefing will be held via Microsoft Teams. Register to attend the industry briefing here.
• A recording of the briefing will also be available to view via eProcure if you are unable to attend the industry briefing session. 

Communication and enquiries 

• All communications and enquiries must only be directed in writing via eProcure. 
• Questions will also be accepted during the industry briefing session.
