Eastern Health: Improving information sharing with GPs project update

EMPHN is pleased to share an exciting update as part of the ongoing work by Eastern Health to strengthen communication between hospital services and primary care.

This work forms part of the broader CareSynergy project, a collaborative initiative proudly led and funded by EMPHN.

As part of this specific project, Eastern Health Specialist Clinics (Outpatients) worked with GPs to co-design improved hospital and specialist clinic event summaries, delivered electronically to GP practice software. This went live on 28 May 2025.

Latest update

Since 29 July 2026, the project expanded to include Eastern Health Endobase scope reports. These reports are now being delivered electronically to GP practice software.

What’s next?

Eastern Health Specialist Clinics (Outpatients) is actively working to: 

  • Improve timely notification of referral status updates
  • Expand Eastern Health notes to GPs including Allied Health and Surgical Operation notes
  • Expand electronic referrals to “at home” services

Why this matters:

Timely, accurate communication between health services and GPs is essential to delivering high-quality patient care. This initiative aims to:

  • Close the communication loop on referrals and outpatient appointments
  • Provide GPs with clear and actionable information
  • Reduce duplication and the need for follow-up calls
  • Support safer transitions of care between hospital and community settings

We look forward to sharing more information, including implementation timelines and support resources.
