Residential Aged Care Resource Navigator

A new free tool to access tailored support for your aged care home.

Eastern Melbourne PHN has launched the Residential Aged Care Resource Navigator – a short, free online tool designed to help residential aged care homes access the right support, without adding to workload. It helps you quickly identify what’s working well, what needs updating, and what may be missing. 

Answer a few simple questions about where your home is at, and you’ll receive a personalised email with practical resources, templates and tools matched to your current needs.  

The Resource Navigator supports: 

  • Telehealth – staff training, policies, equipment and resident support 
  • Digital health – My Health Record and electronic medication charts 
  • After‑hours care – action plans and staff training 
  • Aged Care On‑Site Pharmacist (ACOP) – exploring or implementing the program 
  • GP access – including direct support to help you connect with a GP 

It takes about 5-10 minutes to complete. The resources you receive can also support continuous improvement and help demonstrate alignment with the Aged Care Quality Standards. 

Your responses are confidential and used only to support aged care homes across the EMPHN catchment.
