EMPHN priority projects

At EMPHN, we are embarking on a transformative path to improve the way we collaborate with primary care providers. This evolution is not just a change; it’s a significant shift that will redefine our approach over the next three years.

Our strategy

We understand that not everyone in our region has the same access to healthcare or health outcomes. EMPHN will focus on investing where it is needed most and creating services that ensure everyone in our communities can access healthcare equally. To do this, we will;

  • Use data better and safely to create useful insights that can guide our actions.
  • Support general practices and primary care providers to navigate policy changes.
  • Improve collaboration with partners to create a responsive health system.
  • Enhance the health system and drive better and more equitable outcomes for people in eastern and north-eastern Melbourne.

Our Strategic Plan 2025-2028

Our Strategic Plan 2025-28 sets out our strategic intent, priorities and the outcomes we aim to achieve over the coming years.
It is an ambitious plan that reflects our appetite to do more, to better improve the health system for the people and communities in eastern and north-eastern Melbourne.
