New tender opportunity: Right Care Better Health (RCBH) North program

EMPHN wishes to advise of a new tender opportunity for the Right Care Better Health (RCBH) program in the north of our catchment.

Tender release date: 3.45pm 15 January 2024

Tender briefing: 12pm 22 January 2024

Tender closes: 2pm 19 February 2024

Service delivery commences: April 2024 (indicative)

EMPHN is seeking a Contractor to deliver care coordination activities according to an agreed model of care.

This model of care, Right Care Better Health, aims to improve client management of chronic and complex disease.

The selected provider will enable a nurse care coordinator to work within 10 general practices in the LGAs of Whittlesea, Murrindindi and Mitchell to:

  • identify clients
  • provide care coordination
  • upskill and support sustainability opportunities to participating practices.

The provider will also be responsible for engaging and contracting 10 general practices from which patients will be selected to receive care coordination services for 12 months.

RCBH north provider industry briefing

This session will provide a briefing for potential tenderers for the provision of the RCBH program in the north of the EMPHN catchment.

The industry briefing is your opportunity to learn more about the program, the commissioning and contracting process, to build your understanding of PHNs probity obligations and to learn about the next steps following the tender closure period on 19 February 2014.

The session will be recorded and questions can be asked using the Q&A function. A copy of the recording will be available on eProcure.
