New tender opportunity: Care Navigators Program

EMPHN wishes to advise of a new tender opportunity for the Care Navigators Program in our catchment.

Tender release date: 13 November 2024

Tender briefing: 2pm 19 November 2024

Tender closes: 2pm 13 December 2024

Service delivery commences: March 2025 (indicative)

EMPHN is seeking a provider to deliver care navigation activities in general practices, according to an agreed model of care.

This model of care, the Care Navigators Program, aims to support and resource general practices to provide better mental health care.

The Care Navigators Program is one of the key components of the Mental Health in General Practice Demonstration Model (GP Demo). GP Demo integrates mental health support into GP practices to improve patient care.

The selected provider will enable Care Navigators to work within  60 general practices in the EMPHN catchment to:

  • Identify patients
  • Provide care coordination
  • Support capacity building opportunities to participating practices.

The provider will also be responsible for engaging and supporting service delivery within 60 general practices from which patients will be referred to receive care coordination services.

Tender documentation and notification

The tender will be published on EMPHN’s eProcure online portal. To receive notification when the tender is published, you must be registered on eProcure.

Care Navigators Program industry briefing

This session will provide a briefing for potential tenderers for the provision of the Care Navigators program in the EMPHN catchment.

The industry briefing is your opportunity to learn more about the program, the commissioning and contracting process, to build your understanding of PHNs probity obligations and to learn about the next steps following the tender closure period on 13 December 2024.
