Palliative Aged Care Outcomes Program (PACOP) supports aged care homes (ACHs) to provide optimal palliative & end-of-life care for residents and their families. By embedding a framework of routine assessments to identify increasing palliative and end-of-life needs, PACOP guides care planning to address these needs using an evidence-based practice and evidence-based responses.

PACOP has developed a model of care that routinely monitors all residents, thereby assisting in the early identification of palliative and/or end of life care needs. The model has two components which make up the complete PACOP program:

1. The Profile Collection – routine, regular, sustainable assessments that monitor for any baseline changes in the resident

2. The Outcomes Collection – residents identified with palliative and/or end of life care needs, are transitioned to this part of the program where closer monitoring of the residents means rapid response to distress, symptom management and care planning

This workshop provides an overview of the foundational knowledge required to implement the PACOP Profile & Outcomes Collection within your ACH.

Registration open from 8:30am for a 9am sharp start.

PACOP is a voluntary program funded by the Australian Government Department of Health and Aged Care and is available without cost to all ACHs throughout Australia.

PACOP utilises a Train the Trainer approach to promote ongoing support and sustainability of the program within your ACH. PACOP will provide you with the knowledge and resources you need to support your ACHs whole team to optimise palliative and end-of-life care.
