From 1 July 2025, there will be a change in how health professionals access HealthPathways Melbourne through hospital eReferral forms (Healthlink SmartForms).
This update is important for clinicians who rely on HealthPathways for referral guidance and clinical information.
What’s changing
Starting 1 July 2025, clinicians will need an individual HealthPathways account to continue accessing HealthPathways Melbourne through these SmartForms.
- If you already have an account, simply log in using your existing HealthPathways username and password.
- If you do not yet have an account, you can register for one here: Register for a HealthPathways Melbourne account
Access to HealthPathways Melbourne is available to clinicians working within the Eastern Melbourne PHN and North Western Melbourne PHN catchments.
This change ensures secure, personalised access to clinical content and helps maintain the integrity of referral information shared across the healthcare system.
