The National Allergy Council, has released the National Allergy Transition Standard, together with an accompanying Implementation Guide and Model of Care, developed as part of the National Allergy Council’s Shared Care for Allergy Program.
These resources have been developed in consultation with key interest holders, in response to a well-recognised gap in healthcare during the transition from paediatric to adult services for young people living with allergic conditions. This period is often marked by fragmentation, reduced access to care, and increased risk of adverse health outcomes, particularly for those at risk of anaphylaxis.
The Transition Standard outlines what high-quality, consistent allergy care should look like before, during, and after a young person moves from paediatric to adult services. The accompanying Implementation Guide and Model of Care provides practical steps for embedding transition processes into routine clinical practice, ensuring that services are supported to deliver safe and effective care for patients with allergic disease.
These resources align with national safety and quality priorities and support the delivery of integrated, person-centred care across the health system. For Primary Health Networks, this presents an opportunity to strengthen coordination between primary and specialist care, promote best-practice allergy management within general practice, and support young people and their families to remain engaged with care during and after transition.
