Eastern Melbourne Primary Health Network (EMPHN) invites all general practices to participate in a brief survey on the General Practice Aged Care Incentive (GPACI).
This survey also includes questions related to the Workforce Incentive Program and provides an opportunity to share valuable insights on delivering care to culturally and linguistically diverse communities, as well as individuals experiencing, or at risk of, homelessness.
Your feedback is vital in helping us understand the challenges faced by general practices in supporting these vulnerable populations and improving the quality of care provided to older persons in residential aged care facilities.
The survey will take approximately 10 minutes to complete, and by participating, you’ll go into the draw and have a chance to win 1 of 3 $200 gift vouchers!
We encourage all practices to participate as your feedback will provide valuable insights to help shape future support and resources for your practice.
