A new Coordination Toolkit – A resource for partnerships between RACH and General Practice has been launched to support improved collaboration between Residential Aged Care Homes (RACHs) and General Practices. Designed as a practical resource, the toolkit offers tools and strategies to help strengthen partnerships, streamline planning, and enhance the delivery of primary care services within aged care settings.
Developed as part of the General Practice in Aged Care Incentive Program, the toolkit aims to support a shift from volume-based care to a more structured, preventative model. By promoting regular care planning and greater continuity, the resource is intended to improve the quality of care provided to aged care residents.
The toolkit has been developed for use by staff across both RACHs and primary care settings. It was produced by Primary Health Networks through the National PHN MyMedicare Implementation Program.
