New GP Chronic Conditions Management (GPCCM) MBS items commence from today, 1 July 2025.
What does this mean for your general practice?
- Existing Chronic Disease Management (CDM) Plans and referrals will remain valid until July 2027. There is no need to take immediate action to transition CDM patients to new GPCCMPs.
- Use the new GPCCM MBS item numbers (see table below) in place of the ceased CDM item numbers. When billing any new GPCCM plans or undertaking reviews of a Chronic Disease Management (CDM) plan, use the MBS item numbers for an initial GP Chronic Condition Management Plan (GPCCMP) [965, 392, 29029, 29060]. This is necessary as GPCCMP Review item numbers are not able to be claimed unless a new GPCCMP has been billed. Â
- If you are a MyMedicare practice, encourage any patients with a CDM plan or GP Chronic Condition Management Plans (GPCCMPs) to register with your practice for MyMedicare.
- Explain to them that MyMedicare establishes a unique link to the general practice they wish to see most of the time for ongoing care and management of their chronic health condition/s.
- If a patient does not wish to register for MyMedicare, they should be reassured MyMedicare is voluntary and that they can continue to access GPCCMP care through their usual practice.
- Be sure to schedule regular CCM reviews at the conclusion of each CCM new or review appointment, and clearly explain the care planned for the next review to each patient.
- Practice nurses, Aboriginal and Torres Strait Islander Health Practitioners, and Aboriginal Health Workers can assist the GP or Prescribed Medical Practitioner to prepare or review a GPCCMP
- Team Care Arrangements are no longer required and referral arrangements to allied health providers have been simplified.
- Check MyMedicare registration of your patients prior to scheduling CCM plans. Patients registered for MyMedicare at another general practice are not eligible for CCM MBS items at your practice.
