The Mental Health & AOD Integrated Care Pilot Project focused on improving integrated care for individuals with co-occurring mental health and alcohol and other drug needs over the last year.

The project aimed to enhance care practices, empower staff, and increase collaboration among services while involving lived and living experience in service design and delivery.

  • Integrated care objectives
    The project sought to embed responsive care practices, empower staff through self-assessment and action planning, enhance collaboration and capability across services, and elevate lived and living experience engagement.
  • Program activities and outcomes
    Activities included forums, self-assessments, staff surveys, governance through leadership teams, client journey mapping, participatory research, and presentations.
  • Target groups and partnerships
    The program served individuals with co-occurring mental health and AOD needs, their carers and families, staff across multiple teams at Access Health and Community and Holstep Health, as well as peer workers and individuals with lived experience. Key partners included Access Health and Community as lead agency and Holstep Health as a partner.
