EMPHN invites your general practice to participate in our CCM Activation Series in preparation for changes to Chronic Conditions Management (CCM) MBS items. Chronic Conditions Management (CCM) MBS item changes recommended by the MBS Review Taskforce are the first major change to chronic disease management in 20 years, and are scheduled to come into effect 1 July 2025.Â
Join us on a journey of small manageable changes that aim to maximise the benefits of MyMedicare for your general practice and your patients, strengthening the relationships that patients have with your practice and care providers.
Activity 2 aims to raise awareness among your practice team of MyMedicare and Chronic Conditions Management changes, and support your team to explore and document their roles in both MyMedicare and Chronic Conditions Management.

Stay tuned for Activity 3 which will focus on MyMedicare patient engagement and registration, aiming to raise patient awareness, and have your practice identify what MyMedicare means for your practice team and patient care, so you can communicate this to patients. This activity will also encourage registration, particularly digital MyMedicare Registration, aiming to support your patients to use digital tools that both connect them with MyMedicare Registration and MyHealthRecord, ultimately promoting self-management.
