Eastern Melbourne PHN (EMPHN) is proud to share our 2024–25 Annual Report: Charting progress, driving impact.

This year’s report showcases the collective achievements of our staff and local healthcare providers, as we work together to improve health outcomes across eastern and north-eastern Melbourne.

Key highlights from 2024–25:

• Strategic plan launched: Our 2025–2028 Strategic Plan focuses on driving equitable access, enabling integration, and leveraging insights to improve outcomes for the community.
• Community impact: EMPHN supported the diverse and growing catchment, with targeted programs across Aboriginal and Torres Strait Islander health, mental health, chronic disease and many more.
• Innovative programs: We piloted new models of care like CareSynergy and Link-me+, and strengthened partnerships with local health providers.
• Supporting primary care: EMPHN delivered training, digital enablement, and quality improvement initiatives to help support general practices.
