As care needs within residential aged care become increasingly complex, providers and clinicians are navigating growing demands, system pressures, and evolving models of care.
To support the catchment, EMPHN is launching a series of Aged Care Collaborative Sessions May this year (2026) to bring together primary care, residential aged care, and key system partners to strengthen better outcomes for residents across the Eastern Melbourne region.
These sessions are designed to be practical and solutions‑focused, providing a structured forum for attendees to share what’s working locally, explore common challenges, and identify where targeted support is needed to improve coordination of care.
Attendees will also hear insights from representatives across the health system — including the Commonwealth, local health networks, Ambulance Victoria, the Victorian Virtual ED, and Residential In‑Reach teams. These perspectives will help build a shared understanding of the sector, and shape collaborative approaches to improve outcomes for residents, clinicians and services.
These sessions will cover
- Practical actions that can be implemented now
- Local, real‑world communication approaches
- Co‑design of a virtual Community of Practice
- Agreed next steps to support improved outcomes for residents and clinicians
