Eastern Melbourne PHN (EMPHN) is hosting a series of Aged Care Collaborative Sessions in May 2026, bringing together Residential Aged Care Homes (RACHs), general practice and key system partners to support more coordinated care for residents.
These practical, solutions‑focused sessions provide an opportunity to share what is working locally, explore common challenges, and identify actionable improvements to communication and coordination across the aged care system.
What the sessions will cover
- Practical actions that can be implemented now
- Local, real‑world communication approaches
- Co‑design of a virtual Community of Practice (CoP)
- Agreed next steps to support improved outcomes for residents and clinicians
Who should attend
- RACH managers and clinical leads
- General practitioners
- Practice nurses with an aged care focus
RACHs and GPs are encouraged to attend together to support collaborative discussion and shared solutions.

Registration starts at 8:00am and all sessions are between 8:30–11:30am.
