Eastern Health has updated the way GPs refer to its specialist palliative care clinics. Referrals now go through HealthLink, and the Specialist Palliative Care Services page on HealthPathways has been updated to reflect this change.
Eastern Health’s palliative care clinics provide access to outpatient palliative and supportive care services. Clinic teams work alongside a person’s existing care team, which may include their GP, specialists and community palliative care providers such as Eastern Palliative Care.
Clinics available
- Oncology palliative care clinics (Box Hill and Maroondah)
- Noncancer palliative care clinic (Maroondah)
- Some clinics also offer acupuncture as an adjunct to symptom management where appropriate
What the clinics cover
- Goals of care and future or end-of-life care planning
- Symptoms secondary to an underlying chronic, life-limiting condition (this is broad and may include dyspnoea, pain, nausea, vomiting and psychosocial distress)
- Patients with advanced, life-limiting conditions, including advanced or metastatic cancer, end-stage lung disease, end-stage heart disease, advanced kidney disease, end-stage liver failure, and advanced neurodegenerative diseases or frailty
