From June 2025, referrals to Palliative Care South East and Peninsula Home Hospice will be available through the Consultmed eReferral platform.
Consultmed integrates with Best Practice and Medical Director, streamlining the referral process to save time, improve accuracy, and reduce administrative burden. This secure and trackable system will help minimise delays and enhance patient access to care.
Clinicians can register now at app.consultmed.co to prepare ahead of the rollout.
This change is relevant for general practices within the EMPHN catchment that refer patients to either Palliative Care South East or Peninsula Home Hospice.
