Dementia Action Week is a campaign that runs every year from 15 to 21 September and recognises the impacts of dementia and encourages people to reach out to loved one.
This years theme is “Nobody can do it alone”, which underscores the vital role many people play in supporting those living with dementia. This includes health professionals, who play a role in supporting patients, carers, and communities affected by the condition.
HealthPathways Melbourne provides up-to-date clinical guidance, referral pathways, and local service information to assist in early diagnosis, care planning, and ongoing support for patients and carers.
