Celebrating ten years of Eastern Melbourne PHN

Eastern Melbourne Primary Health Network (EMPHN) is proudly celebrating a large milestone this month – ten years of delivering better health outcomes, better health experiences and an integrated health care system.

In 2015, the Australian Government replaced Medicare Locals and Divisions of General Practice across the country to 31 Primary Health Networks (PHNs) to strengthen primary health care and respond to local health needs.

As we mark 10 years since our establishment, it’s a moment to reflect on the journey we’ve taken and the impact we’ve made across the eastern and north-eastern Melbourne communities.

Over the past decade, our goal has remained clear – to improve the health and wellbeing of our communities by driving better access, stronger integration and more equitable outcomes in primary care. The challenges we face today of a fragmented health system to increasing demand and complexity are not new, but they do require innovative, collaborative responses.

At EMPHN, we work hard to build strong foundations and partnerships to connect the health system. Guided by our 2025-2028 Strategic Plan, we will continue to target investment where it’s needed most, co-design services and build capability across the industry.

We continue to connect our providers, health professionals and communities to foster integration and meaningful change, while leveraging insights to improve outcomes, deliver value and demonstrate measurable impact.

Looking ahead, EMPHN remains focused on driving equitable access to health care and improving outcomes for all members of our community.
