About the role

We are seeking a dedicated and proactive Program Facilitator – General Practice Development to join our Primary Care Innovation and Development team. This role focuses on delivering targeted programs and initiatives that support general practices across the EMPHN catchment.

You’ll play a critical role in engaging with primary health care providers to support the implementation of evidence-based improvements, digital health initiatives, and integrated models of care. This is an exciting opportunity to build strong relationships, influence positive change, and drive better health outcomes in the community.

In this role you will:

  • Communicate with health professionals to educate, inform and influence clinical and service outcomes.
  • Develop and implement tailored engagement and support strategies for general practices based on health priorities and specific needs.
  • Support general practices in implementing policy changes, change management and continuous quality improvement.
  • Encourage and support practices to achieve and maintain current primary health care accreditation standards.
  • Promote the adoption of digital health initiatives, such as Telehealth, e-prescribing, and data extraction technologies.
  • Enhance care team communication and support practices with the effective use of technology.
  • Facilitate integration and collaboration between healthcare providers including general practices, pharmacies, aged care facilities, and allied health.
  • Monitor and evaluate engagement initiatives to measure program effectiveness and inform ongoing improvements.

About you

Tertiary qualifications in nursing, health science or a related discipline are essential.

Experience working in general practice or a related field, with a nursing background highly regarded.

Excellent communication and facilitation skills, with the ability to educate and influence clinical practice.

Understanding of primary care settings, including general practice, pharmacy, and aged care.

Stakeholder engagement skills – Experience working with GPs, practice managers, PHNs, or health organisations.

Understanding of account/territory planning, and quality improvement methodologies.

Flexible and agile mindset with a commitment to continuous improvement.

Strong organisational and project management skills, with the ability to prioritise and meet deadlines.
