About the role:

The Program Facilitator is responsible for leading specific programs, projects, and key focus areas to deliver outcomes across the EMPHN catchment. This role requires leveraging expertise, individual leadership, contractor and stakeholder management, in line with EMPHN’s strategy, transformation agenda, and business plan.

Key Focus Areas:

  • Aged Care
  • General Practice Improvement
  • Palliative Care

Program Facilitation:

  • Plan and facilitate contract management and community of practice meetings with providers.
  • Critically analyze data to inform solutions and identify improvements for all programs/projects.
  • Support the delivery of planned outcomes by providers, including program development, evaluation, and continuous improvement.
  • Drive program and project plans through effective facilitation of commissioned or directly delivered programs.
  • Support an integrated approach to program and project delivery in line with approved plans.
  • Provide contemporary knowledge and expertise in relevant focus areas for successful program and project delivery.
  • Work with internal EMPHN teams across the commissioning cycle to facilitate program delivery, including needs assessments, solution design, procurement, contracting, and evaluation.
  • Utilize community and stakeholder data to build relationships, inform strategy, shape services, build capacity, and improve health outcomes.
  • Support EMPHN’s participation in Primary Care Collaboratives and other relevant working groups, collaborations, or networks.
  • Engage in informed and timely two-way communication across teams to contribute to outcomes and maintain continuous improvement aligned with program and business plans.

Qualifications and Experience:

  • Demonstrated experience in program facilitation, contract management, and stakeholder engagement.
  • Relevant tertiary qualifications in health or related discipline are preferred.
  • Expertise in one or more key focus areas: Aged Care, General Practice Improvement or Palliative Care.
  • Strong analytical and problem-solving skills.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively across teams and with external stakeholders.
  • Knowledge of the commissioning cycle and integrated program delivery.
