Healthcare collaboratives
Primary healthcare collaboratives are region-wide platforms of service providers and organisations working towards shared goals, such as the management of chronic disease and complex conditions in people at risk of poor outcomes.
These collaboratives bring about improved alignment of primary and secondary service providers, with a shared objective of slowing disease progression. In turn, this can prevent deterioration and reduce avoidable hospital admissions through improved community-based models of care.
Our healthcare collaboratives
Currently, we facilitate two primary healthcare collaboratives and three mental health collaboratives.
Primary healthcare collaboratives

Mental health collaboratives
- Eastern Mental Health Service Coordination Alliance (EMHSCA)
- North Eastern Mental Health Service Coordination Alliance (NEMHSCA)
- Eastern Regional Coordinators (ERC)
We are also working collaboratively with stakeholders in Northern Health and Monash Health service areas through existing structures. Both of these collaboratives, and our work in these areas, will grow and develop over time.
