Aged Care GP Matching
Supporting connection between residential aged care homes and GPs
Eastern Melbourne Primary Health Network (EMPHN) is facilitating connections between general practitioners (GPs) and residential aged care homes (RACHs) to support residents who do not currently have access to a regular visiting GP or who may be losing access due to retirement etc. The Aged Care GP Matching Service is designed to strengthen access to high‑quality primary care in aged care settings and support safe, coordinated and person‑centred care for older people.
By supporting collaboration between general practice and residential aged care, EMPHN is helping to address gaps in GP availability, improve continuity of care, and support sustainable models of care across the Eastern Melbourne region.
Who can express interest?
This service is intended for both residential aged care and general practice. You are encouraged to submit an expression of interest if you are:
- A RACH staff member seeking to establish or increase regular GP services for residents
- A GP or general practice interested in providing care to residents of aged care homes or expanding existing services
The service supports both new and existing relationships, particularly where consistent GP access has been challenging or where services are looking to strengthen current arrangements.
