Clinical Insights: Resource Hub
This hub supports general practitioners (GPs) who are participating in the Clinical Insights Program – Eastern Melbourne PHN’s new data-driven feedback for GPs. The program provides personalised clinical feedback to drive improvements in patient care.
Clinical Insights is an evidence-based initiative developed with GPs, for GPs. It focuses on priority clinical behaviours within the control of general practice, such as prescribing and test ordering based on:
- Identified practice gaps and variations in care
- Current evidence-based guidelines
- Clinical software data captured in POLAR
- GP interest and relevance to health priorities
These materials are designed to support the review of Clinical Insights reports, preparation for mini audits, and delivery of high-quality care, particularly for patients at risk of Chronic Kidney Disease (CKD) and those requiring secondary prevention of Cardiovascular Disease (CVD) following stroke or TIA.
