Attendees will gain insights into how these programs enhance care coordination, improve access to essential health information, and support better health outcomes for aged care residents.

My Health Record Benefits for Residential Aged Care Teams
- Improved access to medication history
- Greater understanding and visibility between healthcare professionals involved in patient health and care.
- Enhanced community of care for patients and enhanced capacity to better coordinate care and provide support for patients and their carers
MyMedicare is a new voluntary patient registration model. It aims to formalise the relationship between patients, their general practice, general practitioner (GP) and primary care teams. Evidence shows that seeing the same GP and healthcare team regularly leads to better health outcomes.
The General Practice in Aged Care Incentive (GPACI) will make it easier for older people living in residential aged care homes to receive regular visits and care planning services from their responsible GP and practice.
