Link-me+ is impacting how mental health care is identified and supported in general practice.

Join fellow GPs, Practice Managers and Practice Nurses for an important breakfast conversation bringing together clinical experiences, patient perspectives and emerging evidence from the model.

Learning outcomes

  • Outline how the Link-me+ model supports evidence-informed and stepped mental health care
  • Explain how GPs and Care Navigators can work collaboratively to provide coordinated, person-centered mental health care
  • Identify practical strategies to address implementation barriers and strengthen the integration of Link-me+ in general practice

Who should register?

All GPs and general practice staff across the EMPHN catchment are welcome, whether you’re currently using Link-me+ or simply interested in learning more. No prior experience with the program is required.

Places are limited, so we encourage you to register early to secure your spot.
