Eastern Health invites GPs to an upcoming engagement session to hear the latest updates on the CareSynergy Project and share feedback on improving event summaries sent to your practice management system.

The project aims to improve information transfer between Eastern Health and local General Practices, enabling safer, more connected care for our shared patients.

  • Focuses on closing the communication loop on referrals and outpatient appointments and providing GPs with clear and actionable information.
  • Plans to improve timely notification of referral status updates
  • Expand electronic referrals to “at home” services
  • Electronically deliver event summaries to GP practice software (and to My Health Record) for specialist clinics (outpatients) appointments, allied health appointments, surgical operation notes, endoscopy reports.
