Join Our Community of Practice for Palliative Care in General Practice!

The Eastern Melbourne Primary Health Network (EMPHN) is excited to invite passionate general practitioners (GPs) and GPs in training to join our Community of Practice for Palliative Care in General Practice.

Partnering with the Eastern Metropolitan Region Palliative Care Consortium (EMRPCC) and Eastern Palliative Care, this initiative aims to elevate best practices and enhance palliative care capabilities within our community.

This session will focus on palliative care in RACH, including role of palliative care for those living with dementia. The presentation will be delivered by Dr Aaron Wong, the clinical director of the palliative care service at Eastern Health. Dr. Wong is also a dual trained geriatrician and palliative care physician.

Session summary: In Australia, more than half of permanent residents in aged care facilities are living with dementia. A significant proportion of these residents have unmet palliative care needs and often experience inadequate pain control, fewer palliative care intervention and increased hospital admissions which may be burdensome and distressing. These facilities face multifaceted challenges in managing advanced dementia, including limitations in staffing capacity and confidence, complexity in clinical need, care fragmentation and barriers to accessing palliative care. Effective and timely palliative care is essential in avoiding unwanted and distressing hospital admissions, supporting preference-based care and improving quality of life. Given that GPs provide the majority of the medical care for this large and increasing group of people, GP education and support for delivery of palliative care is essential in reducing this unmet need. This presentation will explore the principles, challenges and support options including novel models of care for optimising the delivery of palliative care for residents living with dementia.
