Community palliative care serves as a vital element in improving the quality of life for individuals dealing with serious illnesses and their families. It provides the necessary compassionate support and specialised medical care to manage pain, symptoms, and emotional distress outside the confines of a hospital.

In this context, GPs play a pivotal role as the main point of contact, delivering consistent care and personalised assistance. GPs work in collaboration with the palliative care team to address the physical, emotional, and spiritual needs of patients, ensuring they can make the most of their valuable time in familiar surroundings while receiving comprehensive and holistic support.

Learning outcomes:

  • Demonstrate an understanding of the collaborative processes between interdisciplinary Community Palliative Care services and GP’s, aiming to enhance the quality of care for individuals in their end-of-life stages while being cared for at home
  • Identify specific eligibility requirements necessary for involvement in the accurate completion of the Medical Certification of Cause of Death
