Join our information session for Residential Aged Care Home staff.

Attendees will gain insights from guest speakers into how advance care planning supports quality end-of-life care and how My Health Record can be effectively used to document and access this information.

Learning Objectives:

  • Describe the importance of advance care planning and how this plays an important role in promoting quality care at the end-of-life.
  • Describe the importance of how to develop effective communication skills to engage in meaningful conversations with residents and their families about advance care planning.
  • Understand the advance care planning documentation and how to document advance care planning conversations in My Health Record.
  • Gain a better understanding of My Health Record and how it can help in Residential Aged Care Homes in delivering quality patient care.
