SUMMARY:Foundations in palliative care for general practice

They will also learn about local community palliative care providers and the services they offer. \n\n\n\nPalliative care is person- and family-centred treatment\, care and support for people living with a life-limiting illness. Primary care providers play a crucial role in providing this care in the community\, ensuring that people with a life-limiting illness and their families get the support and care needed to live well.  \n\n\n\nThe event will feature a local general practitioner\, detailing experiences providing community palliative care\, as well as outlining the aims\, activities and responsibilities involved with providing palliative care in a general practice setting. Attendees will also hear from three community palliative care providers in Melbourne’s north west\, who will provide an overview of their services. They will discuss how to identify patients for whom palliative care is appropriate and reveal practical tips on supporting patients through advanced care planning. \n\n\n\nSpeaker Bios \n\n\n\nDr Haoming Zhou is a geriatrician and palliative medicine physician working across Melbourne through Northern Health and Melbourne City Mission Palliative Care. He has joined the Northern Advance Care Planning team to continue to encourage discussion and understanding to promote person-centred care through shared decision-making. His work at Melbourne City Mission Palliative Care is aimed at improving access to specialist palliative care in residential aged care. He is involved in teaching and supervision and is interested in listening and learning from the stories we all have and share. \n\n\n\nDr Terry Ahern is a semi-retired GP at MyHealth Brunswick. He is passionate about care of the elderly\, aged care\, palliative care and chronic disease management. He is a member of North Western Melbourne Primary Health Network’s Older Adults Expert Advisory Group. As well as seeing elderly and frail patients in practice\, Terry regularly visits them at residential aged care homes and provides palliative care. \n\n\n\nDr. Chien-Che Lin is a palliative medicine specialist consultant at several hospitals and community palliative care services in Victoria\, including Banksia Palliative Care Service. He also works as a general practitioner. Committing to support people with progressive\, incurable disease to live as well as possible with the burden of diseases and dying\, he has continuously dedicated clinical and administrative commitments to improve care in the community. He is actively involved in teaching and supervision of medical students and medical specialist trainees\, aiming to inspire a practice culture of both academic excellence and human compassion across all clinical settings. \n\n\n\nDr Annie Chiu is the head of the palliative care service at Werribee Mercy Hospital and Mercy Palliative Care. She obtained the Palliative Medicine fellowship in 2016 and has worked in inpatient and community palliative care settings since. She has a special interest in non-malignant palliative medicine and leads the hospital’s palliative clinic. The clinic provides symptom management and supportive care to patients with a life-limiting non-cancer illness\, such as end-stage COPD\, heart failure\, or end-stage kidney disease. \n\n\n\nDr Chiu is an honorary lecturer at the University of Notre Dame and is also heavily involved in junior medical officer training and teaching. DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nEastern Melbourne Primary Health Network (EMPHN) runs training sessions on the IAR-DST for GPs\, general practice staff\, clinicians and other mental health service providers. \n\n\n\nThe training consists of a two-hour workshop delivered online with a focus on: \n\n\n\n\nIntroduction to the IAR-DST\n\n\n\nOrientation to the domains\, levels of care and decision support tool\n\n\n\nClinical judgement and supported decision-making\n\n\n\nApplication of the tool in assessment and intake settings (practical activity using a scenario)\n\n\n\n\nThe training is an RACGP CPD Approved Activity for the 2023–2026 triennium. \n\n\n\nEligible GPs who complete the training can claim a once-off payment of $300 (excluding GST).

Originally scheduled to commence on November 1 2024\, the additional time will support practices and providers\, including GPs and allied health providers\, to be ready for the changes to these important services. Information about the changes to the chronic disease management framework and the amended timeframe can be found on DoHAC’s website. As a result of this decision\, the webinar will be postponed. We will reschedule this event next year in advance of the new implementation date for the changes. \n\n\n\nRACGP are holding a webinar in October about the upcoming changes to chronic disease management items. This webinar will be very timely ahead of the changes commencing (pending passage of legislation) from 1 November. All General Practices\, GPs\, Practice Managers and other Practice staff are encouraged to attend. \n\n\n\nA revised framework for chronic disease management will commence on 1 November 2024. These changes\, which were announced in the 2023-24 Budget\, reflect the recommendation of the MBS Review Taskforce. \n\n\n\nKey changes include: \n\n\n\n\nReplacing GP Management Plans and Team Care Arrangements with a single GP Chronic Condition Management Plan.\n\n\n\nRemoval of the requirement to consult with at least two collaborating providers\, as described under the current Team Care Arrangement.\n\n\n\nProviding for practice nurses\, Aboriginal and Torres Strait Islander health practitioners and Aboriginal heath workers to assist with the preparation and review of plans.\n\n\n\nSupporting continuity of care by requiring patients registered through MyMedicare to access plans and reviews through the practice where they are registered.\n\n\n\nEqualising the fees for development and review of a plan to support ongoing reviews.\n\n\n\nMoving to referral letters for referrals to allied health services.\n\n\n\nA two year transition period for existing patients with a GP Management Plan and/or Team Care Arrangement.\n\n\n\n\nLearning outcomes: \n\n\n\n\nUtilise referral letters to provide relevant clinical information to allied health professionals.\n\n\n\nIdentify new MBS item numbers for chronic condition management and when to bill these.\n\n\n\nImplement arrangements for other healthcare professionals to assist with the preparation and review of GP chronic condition management plans.\n\n\n\n\nSpeakers: \n\n\n\n\nImogen Colton – Director of the General Practice Section in the MBS Policy and Review Branch at the Department of Health and Aged Care\n\n\n\nLouise Riley – Assistant Secretary of the MBS Policy and Review Branch of the Australian Government Department of Health and Aged Care

Learn how to take appropriate collateral history\, using cognitive assessment tools to help determine fitness to drive and reassess driving capacity in people with dementia.  \n\n\n\nDriving safely requires a wide range of manual and cognitive skills. Dementia can affect driving ability in many ways with changes often starting subtly but getting worse over time.  \n\n\n\nGeneral Practitioners are often front and centre when the driving ability of a person living with dementia is in question. At this webinar you will hear practical tips from expert GP medical educators about the importance of beginning this conversation early\, involving family and care-partners in the process\, appropriate use of cognitive assessment tools and ongoing reassessment.  \n\n\n\nThis event is approved for 1.0 hours of Educational Activities for RACGP. \n\n\n\nLearning outcomes: \n\n\n\n\nRecall frameworks helpful in assessing and caring for people living with cognitive impairment and dementia \n\n\n\nExplain the impact that dementia can have on a person’s driving ability\n\n\n\nApply national driving guidelines when assessing a person’s ability to drive.\n\n\n\n\nSpeaker information: \n\n\n\nDr Karen Savery is a Brisbane based GP who works in clinical practice\, residential aged care and medical education. She is passionate about the education and mentoring of GPs\, particularly in the area of aged care and dementia. She has worked with Dementia Training Australia to increase GP’s confidence in the recognition and management of dementia.  \n\n\n\nDr Steph Daly is a specialist GP in Dementia. She is a fellow of the Royal College of General Practitioners and has completed post-graduate training in Dementia Studies at the University of Bradford in the UK.  More recently since moving to Australia she has become an advocate for patients living with Dementia and has facilitated the practice she works at becoming one of the first dementia-friendly (Dementia Australia approved) GP surgery’s in Australia.
