DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nEMPHN is hosting a series of Aged Care Collaborative Sessions across Eastern Melbourne. These sessions are designed to provide a practical forum for clinicians\, aged care providers and system partners to come together\, share local experience\, and work through common challenges.Sessions will focus on what is working in practice\, where gaps or barriers exist\, and what practical actions can be taken to improve communication\, coordination and care outcomes. Participants will also hear perspectives from across the health system\, including the Commonwealth\, local health networks\, Ambulance Victoria\, the Victorian Virtual ED and Residential In‑Reach teams\, to support shared understanding and alignment. \n\n\n\nRACHs and GPs are encouraged to attend together to support collaborative discussion and shared solutions.
