Each session is designed to support shared learning\, strengthen local connections and explore practical approaches to improving coordination of care for residents. \n\n\n\nWho should attend\n\n\n\nThis session is suitable for: \n\n\n\n\nGeneral practice staff\, particularly GPs providing care to residents in RACHs\n\n\n\nPractice nurses and nurse practitioners supporting aged care work\n\n\n\nRACH staff\, including clinical managers\, facility managers\, and relevant corporate roles\n\n\n\nOther healthcare providers involved in delivering care to residents in RACHs\n\n\n\n\nWhat to expect\n\n\n\nEach quarter will have a different theme.
