DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nEastern Health invites GPs to an upcoming engagement session to hear the latest updates on the CareSynergy Project and share feedback on improving event summaries sent to your practice management system. \n\n\n\nThe project aims to improve information transfer between Eastern Health and local General Practices\, enabling safer\, more connected care for our shared patients. \n\n\n\n\nFocuses on closing the communication loop on referrals and outpatient appointments and providing GPs with clear and actionable information.\n\n\n\nPlans to improve timely notification of referral status updates\n\n\n\nExpand electronic referrals to “at home” services\n\n\n\nElectronically deliver event summaries to GP practice software (and to My Health Record) for specialist clinics (outpatients) appointments\, allied health appointments\, surgical operation notes\, endoscopy reports.\n\n\n\n\nSession Details:\n\n\n\n\n1:00 pm – 2:00pm\, Tuesday 16th June\, Online via Microsoft Teams Meeting\n\n\n\nTopic 1: CareSynergy Project Update\n\n\n\nTopic 2: More event summaries from EMR and VicKey to PMS (MD/BP etc)\n\n\n\nOther feedback\, discussions\, or comments\n\n\n\n\nYou will receive a complimentary gift in appreciation of your time. \n\n\n\nIf you are interested in attending\, please email laura.choi@easternhealth.org.au and lauren.collet@easternhealth.org.au to confirm for this event.
