The Pathways++ Program is a nurse led, Hospital Admission Risk Program (HARP), step down program for adults living with chronic conditions.

Pathways ++ is a model of care coordination and social prescribing for medically stable patients that require longer term management and education around their chronic condition. Pathways++ was showcased at the 2024 24th International Conference on Integrated Care in Belfast, where it received international recognition.
