SUMMARY:Workshop 2: St Vincent's Hospital Melbourne (SVHM) CareSynergy - Barriers to Discharge from Specialist Clinics
DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nSt Vincent’s Hospital Melbourne (SVHM) invites GPs to attend 2 workshops (25th March and 27th May 2025) to explore barriers to discharge from specialist clinics and associated interventions to overcome these barriers. \n\n\n\nIn 2024\, they developed resources to facilitate ‘Supported Discharge’ from specialist clinics for patients with chronic Hep B (not on treatment) and those who had undergone bariatric surgery. These resources drew on established guidelines and input from our gastroenterology and bariatric surgery teams to develop handover tools. \n\n\n\nThis work highlighted the broad range of barriers to discharge\, and served as a prompt for St Vincent’s current project ‘Supported Discharge from Specialist Clinics’: A Study of Feasibility and Anticipated Value\, which is funded by Eastern Melbourne PHN’s CareSynergy Program. CareSynergy is aimed at improving integration between hospitals and primary care across the North East Metro. \n\n\n\nThe SVHM CareSynergy project is undertaking stakeholder consultation (including GP workshops) and other research to identify \n\n\n\n– barriers to discharge from clinics\, and \n\n\n\n– resources/interventions/relationships which overcome these barriers \n\n\n\nWe will then develop a model(s) of care which incorporates these adaptations. \n\n\n\nThe project seeks to promote a sense of collaboration between hospitals and primary care\, contribute to the efficient utilisation of specialist clinics and optimise access for additional patients requiring non-GP specialist assessment/care. \n\n\n\nWorkshop Details: \n\n\n\n\nWorkshop 1 – identifying barriers and proposing solutions\n\n8:30 am – 11 am\, Tuesday 25th of March 2025\n\n\n\nSt Vincent’s Hospital Melbourne\, Fitzroy Campus\n\n\n\n\n\nWorkshop 2 – review of proposed care model(s)\n\n8:30 am – 10:30\, Tuesday 27th of May 2025\n\n\n\nSt Vincent’s Hospital Melbourne\, Fitzroy Campus OR via Microsoft Teams meeting (to be decided)\n\n\n\n\n\n\nYou will be remunerated for your time at a rate of $150/hour (+GST) including 0.5 hours of pre-reading. \n\n\n\nIf you are interested and available to attend\, please reply via rachael.sutherland@svha.org.au or to joint project leads Cath White catherine.white@svha.org.au and Meg Marmo meg.marmo@svha.org.au. for this event.

SUMMARY:Patient preferences matter: how practice nurses can support advance care planning
DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nNWMPHN and Northern Health have partnered to offer practice nurses the opportunity to learn more about the importance of advance care planning and how to incorporate these principles into clinical practice. \n\n\n\nAdvance care planning (ACP) allows people to prepare for a future situation in which they may no longer be able to make their own medical or health care decisions. Practice nurses have the opportunity to begin discussions about this difficult topic and align the goals and preferences of the people they know best. Proactive communication and normalising these conversations are essential. Although some people will not engage\, others will want to know more and even write information down in an advance care directive. This workshop aims to build confidence\, capacity and capability\, and aligns with the message of National Advance Care Planning Week: “Make your choices matter”. There will be opportunities for questions and case discussion about your experiences within the session. \n\n\n\nSpeakers: \n\n\n\nElla Critchley is the program manager of Northern Health ACP. She has over 20 years of experience in the health care industry. Ella is an experienced nurse leader with a strong sense of integrity who effectively and enthusiastically supports clinicians in understanding ACP. Ella She a Master of Healthy Ageing and builds strong\, positive and meaningful relationships.  \n\n\n\nDr Hao Zhuo is a geriatrician and palliative medicine physician working across Melbourne through Northern Health and Melbourne City Mission Palliative Care. Nearly 30 per cent of Australians living with chronic hepatitis B remain undiagnosed\, and therefore at heightened risk of liver failure and cancer. This session will equip your team with practical approaches to enhance early detection and improve patient outcomes. \n\n\n\nThe session will be presented by three experts in the field: viral hepatitis educator Mieken Grant; hepatitis B s100 prescriber Dr Natalia Rode; and practice nurse Tanya Hounslow. \n\n\n\nThey will guide you through the steps of developing a quality improvement plan\, show you tips and tricks for searching your clinic’s patient database\, and suggest implementable strategies to enhance the care you provide your patients who are living with hepatitis B or may be at risk. \n\n\n\nSpeakers \n\n\n\nMieken Grant is a viral hepatitis educator\, a statewide role funded by the Victorian Department of Health. She educates health professionals about hepatitis B and C\, and supports them in delivering best practice viral hepatitis prevention\, testing\, treatment and care.  \n\n\n\nMieken is a Registered Nurse with extensive experience in public health\, concentrating mainly on sexual health and blood-borne viruses. She is passionate about improving the health of marginalised communities. She has worked in public hospitals and sexual health centres\, as well as in community and remote settings\, supporting clients with complex social and medical care needs. Mieken also has experience in policy review\, research\, education\, partner notification and contact tracing. \n\n\n\nDr Natalia Rode is a GP\, researcher and medical educator. She is passionate about quality improvement in general practice. As a hepatitis B s100 prescriber\, she is particularly interested in improving care for people living with this condition. \n\n\n\nTanya Hounslow is a practice nurse at Northside Clinic\, dedicated to making a real difference in patient care through quality improvement activities. Topics include: \n\n\n\n\nNational Lung Cancer Screening Program overview\n\n\n\nEligibility and the screening journey\n\n\n\nThe role of the National Cancer Screening Register\n\n\n\nHow to integrate and register for the Healthcare Provider Portal\n\n\n\nLocal referral pathways for practices\n\n\n\n\nThis event is ideal for General Practitioners involved in lung cancer screening and management. \n\n\n\nThis is a RACGP CPD 1.5 hrs accredited activity. \n\n\n\nGuest Speakers: \n\n\n\nProfessor Dorota Gertig – MB.BS (Hons)\, MHSc\, ScD\, FAFPHM\, GAICD \n\n\n\nSince July 2016\, Dorota has been the Medical Director of the National Cancer Screening Register (NCSR)\, playing a key role in implementation and operations of the Register. She is a public health physician and epidemiologist who completed her medical training at Monash University and has a Doctorate in Epidemiology from the Harvard School of Public Health. Professor Gertig was previously the Medical Director of the Victorian Cervical Cytology Registry and the inaugural Medical Director of NHVPR. \n\n\n\nShe is a respected leader in cancer screening and a member of several key national and state committees on cervical screening\, including the National Quality and Safety Monitoring Committee for the National Cervical Screening Program. Professor Gertig has an international profile in cancer research\, including the epidemiology of cervical\, breast and ovarian cancers. With more than 120 scientific publications\, her primary research focus is on policy relevant research as well as improving participation in cancer screening. \n\n\n\nDr Hashinee Weraduwage – DCH\, MBBS\, FRACGP \n\n\n\nDr Hashinee Weraduwage graduated from the University of Adelaide and completed her GP training in SA and has been working in Melbourne since 2020. She joined the HealthPathways Melbourne team in April 2024 as a Clinical Editor and has reviewed and updated pathways in Gastroenterology\, Obstetrics\, Immunisation\, Haematology and resources pages for Guide to MBS\, Telehealth\, and Medicare.
