SUMMARY:Palliative Aged Care Outcomes Program (PACOP) Fundamentals workshop
DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nPalliative Aged Care Outcomes Program (PACOP) supports aged care homes (ACHs) to provide optimal palliative & end-of-life care for residents and their families. By embedding a framework of routine assessments to identify increasing palliative and end-of-life needs\, PACOP guides care planning to address these needs using an evidence-based practice and evidence-based responses. \n\n\n\nPACOP has developed a model of care that routinely monitors all residents\, thereby assisting in the early identification of palliative and/or end of life care needs. The model has two components which make up the complete PACOP program: \n\n\n\n1. The Profile Collection – routine\, regular\, sustainable assessments that monitor for any baseline changes in the resident \n\n\n\n2. The Outcomes Collection – residents identified with palliative and/or end of life care needs\, are transitioned to this part of the program where closer monitoring of the residents means rapid response to distress\, symptom management and care planning \n\n\n\nThis workshop provides an overview of the foundational knowledge required to implement the PACOP Profile & Outcomes Collection within your ACH. \n\n\n\nRegistration open from 8:30am for a 9am sharp start. \n\n\n\nOther information \n\n\n\nPACOP is a voluntary program funded by the Australian Government Department of Health and Aged Care and is available without cost to all ACHs throughout Australia. \n\n\n\nPACOP utilises a Train the Trainer approach to promote ongoing support and sustainability of the program within your ACH. PACOP will provide you with the knowledge and resources you need to support your ACHs whole team to optimise palliative and end-of-life care.

SUMMARY:Hepatitis C Community of Practice
DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nThis free online training aims to provide participants with the knowledge and confidence to increase the management of HCV in alcohol and other drug settings. \n\n\n\nPeople who inject drugs and experience social and structural inequities such as homelessness are at increased risk of acquiring HCV and are therefore a priority population for HCV models of care. \n\n\n\nHosted by the Victorian HIV and Hepatitis Integrated Training and Learning program\, this Hepatitis C Community of Practice session is intended to support the alcohol and other drugs workforce with the opportunity to bring case based studies for discussion and mutual learning. This peer learning approach enhances the practice of AOD workers to recognise those who may be living with or at risk of HCV and supporting them onto the care pathway\, particularly if they are disengaged with traditional health care services. Linking attendees with expert knowledge in point of care testing\, prison settings\, nurse led models of care and community facing support services\, the Community of Practice aims to support the elimination of Hepatitis C in Victoria. \n\n\n\nAll practitioners are invited to submit cases for discussion. Presenters will be selected based on the needs of each case so early submission is encouraged. \n\n\n\nWhile this is not a clinical update\, a base level of knowledge of hepatitis C will be assumed in the discussion. You can register for the Clinical Foundations of Hepatitis C course via ASHM On Demand here. \n\n\n\nDate: Monday\, 24 March 2025 \n\n\n\nTime: 12:00pm – 1.30pm \n\n\n\nTopic: Homeless Population and HCV \n\n\n\nPresenters: Jacqui Richmond and others TBC \n\n\n\nLearning Outcomes: \n\n\n\n\nIdentify appropriate point of care testing options for people at risk of HCV\n\n\n\nPlan appropriate pathways to care\, from testing to treatment and beyond\n\n\n\nLearn about innovative models of care of service delivery which can be successfully used in marginalised/challenging populations\, including:\n\nincorporation of POC testing\n\n\n\ninitiation of therapy in non-healthcare or outreach settings\n\n\n\nretention in care for challenging populations

SUMMARY:Workshop 1: St Vincent's Hospital Melbourne (SVHM) CareSynergy - Barriers to Discharge from Specialist Clinics
DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nSt Vincent’s Hospital Melbourne (SVHM) invites GPs to attend 2 workshops (25th March and 27th May 2025) to explore barriers to discharge from specialist clinics and associated interventions to overcome these barriers. \n\n\n\nIn 2024\, they developed resources to facilitate ‘Supported Discharge’ from specialist clinics for patients with chronic Hep B (not on treatment) and those who had undergone bariatric surgery. These resources drew on established guidelines and input from our gastroenterology and bariatric surgery teams to develop handover tools. \n\n\n\nThis work highlighted the broad range of barriers to discharge\, and served as a prompt for St Vincent’s current project ‘Supported Discharge from Specialist Clinics’: A Study of Feasibility and Anticipated Value\, which is funded by Eastern Melbourne PHN’s CareSynergy Program. CareSynergy is aimed at improving integration between hospitals and primary care across the North East Metro. \n\n\n\nThe SVHM CareSynergy project is undertaking stakeholder consultation (including GP workshops) and other research to identify \n\n\n\n– barriers to discharge from clinics\, and \n\n\n\n– resources/interventions/relationships which overcome these barriers \n\n\n\nWe will then develop a model(s) of care which incorporates these adaptations. \n\n\n\nThe project seeks to promote a sense of collaboration between hospitals and primary care\, contribute to the efficient utilisation of specialist clinics and optimise access for additional patients requiring non-GP specialist assessment/care. \n\n\n\nWorkshop Details: \n\n\n\n\nWorkshop 1 – identifying barriers and proposing solutions\n\n8:30 am – 11 am\, Tuesday 25th of March 2025\n\n\n\nSt Vincent’s Hospital Melbourne\, Fitzroy Campus\n\n\n\n\n\nWorkshop 2 – review of proposed care model(s)\n\n8:30 am – 10:30\, Tuesday 27th of May 2025\n\n\n\nSt Vincent’s Hospital Melbourne\, Fitzroy Campus OR via Microsoft Teams meeting (to be decided)\n\n\n\n\n\n\nYou will be remunerated for your time at a rate of $150/hour (+GST) including 0.5 hours of pre-reading. \n\n\n\nIf you are interested and available to attend\, please reply via rachael.sutherland@svha.org.au or to joint project leads Cath White catherine.white@svha.org.au and Meg Marmo meg.marmo@svha.org.au. for this event.
