Originally scheduled to commence on November 1 2024\, the additional time will support practices and providers\, including GPs and allied health providers\, to be ready for the changes to these important services. Information about the changes to the chronic disease management framework and the amended timeframe can be found on DoHAC’s website. As a result of this decision\, the webinar will be postponed. We will reschedule this event next year in advance of the new implementation date for the changes. \n\n\n\nRACGP are holding a webinar in October about the upcoming changes to chronic disease management items. This webinar will be very timely ahead of the changes commencing (pending passage of legislation) from 1 November. All General Practices\, GPs\, Practice Managers and other Practice staff are encouraged to attend. \n\n\n\nA revised framework for chronic disease management will commence on 1 November 2024. These changes\, which were announced in the 2023-24 Budget\, reflect the recommendation of the MBS Review Taskforce. \n\n\n\nKey changes include: \n\n\n\n\nReplacing GP Management Plans and Team Care Arrangements with a single GP Chronic Condition Management Plan.\n\n\n\nRemoval of the requirement to consult with at least two collaborating providers\, as described under the current Team Care Arrangement.\n\n\n\nProviding for practice nurses\, Aboriginal and Torres Strait Islander health practitioners and Aboriginal heath workers to assist with the preparation and review of plans.\n\n\n\nSupporting continuity of care by requiring patients registered through MyMedicare to access plans and reviews through the practice where they are registered.\n\n\n\nEqualising the fees for development and review of a plan to support ongoing reviews.\n\n\n\nMoving to referral letters for referrals to allied health services.\n\n\n\nA two year transition period for existing patients with a GP Management Plan and/or Team Care Arrangement.\n\n\n\n\nLearning outcomes: \n\n\n\n\nUtilise referral letters to provide relevant clinical information to allied health professionals.\n\n\n\nIdentify new MBS item numbers for chronic condition management and when to bill these.\n\n\n\nImplement arrangements for other healthcare professionals to assist with the preparation and review of GP chronic condition management plans.\n\n\n\n\nSpeakers: \n\n\n\n\nImogen Colton – Director of the General Practice Section in the MBS Policy and Review Branch at the Department of Health and Aged Care\n\n\n\nLouise Riley – Assistant Secretary of the MBS Policy and Review Branch of the Australian Government Department of Health and Aged Care

Learn how to take appropriate collateral history\, using cognitive assessment tools to help determine fitness to drive and reassess driving capacity in people with dementia.  \n\n\n\nDriving safely requires a wide range of manual and cognitive skills. Dementia can affect driving ability in many ways with changes often starting subtly but getting worse over time.  \n\n\n\nGeneral Practitioners are often front and centre when the driving ability of a person living with dementia is in question. At this webinar you will hear practical tips from expert GP medical educators about the importance of beginning this conversation early\, involving family and care-partners in the process\, appropriate use of cognitive assessment tools and ongoing reassessment.  \n\n\n\nThis event is approved for 1.0 hours of Educational Activities for RACGP. \n\n\n\nLearning outcomes: \n\n\n\n\nRecall frameworks helpful in assessing and caring for people living with cognitive impairment and dementia \n\n\n\nExplain the impact that dementia can have on a person’s driving ability\n\n\n\nApply national driving guidelines when assessing a person’s ability to drive.\n\n\n\n\nSpeaker information: \n\n\n\nDr Karen Savery is a Brisbane based GP who works in clinical practice\, residential aged care and medical education. She is passionate about the education and mentoring of GPs\, particularly in the area of aged care and dementia. She has worked with Dementia Training Australia to increase GP’s confidence in the recognition and management of dementia.  \n\n\n\nDr Steph Daly is a specialist GP in Dementia. She is a fellow of the Royal College of General Practitioners and has completed post-graduate training in Dementia Studies at the University of Bradford in the UK.  More recently since moving to Australia she has become an advocate for patients living with Dementia and has facilitated the practice she works at becoming one of the first dementia-friendly (Dementia Australia approved) GP surgery’s in Australia.

In a team-based approach\, utilising pharmacy\, practice nurses\, GPs\, and occupational therapy there are numerous interventions which can improve safety\, palatability and efficacy of medication use in dementia. \n\n\n\nIn this webinar you will hear practical tips from expert GP and pharmacist Medical Educators about managing medication in a person living with dementia\, the importance of medication review and the detrimental effects of certain medications on cognition. He is a long-standing member of Dementia Training Australia’s GP education team. \n\n\n\nHilton facilitates reflective writing workshops for doctors and other health professionals with the goal of deepening their compassion\, overcoming professional isolation and reducing risk of burnout. The workshops have been adapted for people living with chronic and mental illnesses\, as well as enthusiastic amateur writers. Hilton has been invited to present his workshops all the way from Byron Writers Festival to Harvard Medical School. \n\n\n\nHilton’s book\, One Curious Doctor: A Memoir of Medicine\, Migration and Mortality\, explores the personal impact of working as a country doctor. His play\, Enduring Witness\, is used to facilitate conversations about end-of-life care. DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nEastern Melbourne Primary Health Network (EMPHN) runs training sessions on the IAR-DST for GPs\, general practice staff\, clinicians and other mental health service providers. \n\n\n\nThe training consists of a two-hour workshop delivered either online with a focus on: \n\n\n\n\nIntroduction to the IAR-DST\n\n\n\nOrientation to the domains\, levels of care and decision support tool\n\n\n\nClinical judgement and supported decision-making\n\n\n\nApplication of the tool in assessment and intake settings (practical activity using a scenario)\n\n\n\n\nThe training is an RACGP CPD Approved Activity for the 2023–2026 triennium. \n\n\n\nEligible GPs who complete the training can claim a once-off payment of $300 (excluding GST).

Wesley Lifeforce suicide prevention training equips participants with the knowledge and skills to increase awareness of suicide in Australia\, identify the signs that someone may be at risk of suicide and appropriate action to take. Effective interventions can make a real difference and save lives. Wesley Lifeforce programs aim at training individuals from all walks of life\, as well as organisational groups. All Wesley LifeForce facilitators are accredited trainers and have completed suicide intervention training. Below you will find an outline of the workshops. \n\n\n\nThe outcomes \n\n\n\n\nUnderstand risk and protective factors\n\n\n\nHow to identify the warning signs that someone may be at risk of suicide\n\n\n\nHow to ask directly about suicide intent\n\n\n\nUnderstand what local and national support and resources are available to support a person at risk\n\n\n\n\nEnsuring participant safety \n\n\n\nWorkshop material is specifically designed keeping participant’s safety in mind. The course material is vetted by MindFrame – an Australian Government body which provides expert advice to the suicide prevention sector on responsible reporting/portrayal of the issue of suicide.

SUMMARY:General Practice in Aged Care Overview webinar
DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nAs of 1 July 2024\, eligible GPs\, including sole providers can start participating in the General Practice in Aged Care Incentive. This incentive provides GPs and practices with financial incentives for delivering a quality bundle of care including regular visits and care plans to patients that live in a residential aged care home as part of MyMedicare. Incentive payments of $300 per patient\, per year\, to the responsible providers and $130 per patient\, per year\, to practices are available to participating GPs and general practices\, who provide services to each of their MyMedicare registered patients in a residential aged care home rather than at their practice. These payments will be made quarterly and are in addition to the MBS or DVA rebates and the MyMedicare benefits. To help GPs understand this new incentive and what it means for them\, the RACGP is hosting a webinar presented by the Department of Health and Aged Care. The webinar will provide an overview of the General Practice in Aged Care Incentive\, how GPs can make the most of the new incentive and how GPs can keep on top of their obligations under the scheme.
